Mikosch ================================================================= http://www.sas-centre.org/_new/site/assays/bone_markers/ntelopep.html Analytical information NTx: The assay measures N-telopeptide linked pyridinoline and deoxypyridinoline residues from the breakdown for type 1 collagen in human urine. The antibodies do not recognise non-pyridinoline-linked peptides for free pyridinoline. The assay is a competitive inhibition EIA utilising a monoclonal antibody labelled with horse radish peroxidase. The assay is standardised against a collagenase digested bone collagen and reported in terms of the ratio of bone collagen equivalents (nmol/L) to creatinine concentration (mmol/L). The coefficient of variation for the assay is 7% across the reference range. Biological variation: There is a significant circadian rhythm with higher levels of excretion at night (as much as 50%). This rhythm is the same throughout life, but the form changes in osteoporosis. There is also a day to day variation in excretion of N-telopeptide (quoted as greater than 24%). Patient preparation: I it is recommended that a carefully monitored second void urine collection is made; patients should be asked to empty their bladders on waking and discard the urine. Then they should collect the next specimen (the second morning void) in the container (plain container, no additives). To minimise the day to day variability in serial measurement it is important to maintain the same collection protocol. Stability: The NTx levels in urine are reasonably stable; it is recommended that urines be stored at 2-8C for up to 7 days, but deep frozen at -20C if longer storage is expected. It is recommended that a sample stored at -20C should not be subjected to more than 5 freeze/that cycles. Furthermore, it is recommended that samples should not be stored at any stage in direct sunlight. It is recommended that samples should be collected without preservative and therefore storage at 2-8C is preferred to avoid contamination. There is data to suggest that NTx levels are more variable where samples are collected with a preservative possibly due to precipitation of other components in urine at 2-8C. Horseradish peroxidase로 결합된 Monoclonal antibody가 환자 검체의 NTx와 Microwell에 흡착된 NTx를 경쟁적으로 결합한다. 현재 임상적 유용성이 밝혀진 bone metabolism의 생화학적 지표물질로는 Bone formation에 관여하는 Serum bone-specific alkaline phosphatase, Serum osteocalcin, Serum propeptide of type I procollagen과 Bone resorption에 관여하는 Urine and serum cross-linked N-telopeptide, Urine and serum cross-linked C-telopeptide, Urine total free deoxypyridinoline, Urine hydroxyproline, Serum tartrate-resistant acid phosphatase, Serum bone sialoprotein 및 Urine hydroxylysine glycosides 등이 알려져 있습니다. NTx는 (Cross-linked N-teleopeptide of type I collagen)은 Bone resorption marker로 urine이나 serum을 이용하여 ELISA format이나 Point-of-Care (POC) device를 이용하여 쉽게 측정할 수 있습니다.
